Slip and Fall Accident Checklist

Many people are extremely embarrassed when they slip and fall.  This embarrassment often compels a person to leave the scene of an accident.  If you have fallen and especially if you feel injured as a result, leaving the scene of an accident is the worst thing you can do.  Of course, this may be impossible for you if you are whisked away in an ambulance, but keep in mind the importance of collecting information from where the injury occurred.

There are certain types of evidence our office needs in order to zealously represent you.  If you suffer an injury from a slip and fall accident, gathering the following information before you leave the scene of the accident will greatly benefit your cause.


  • Take pictures of where you fell and the surrounding area to determine if the floor was wet and if there were any signs nearby
  • Take pictures of all areas of clothing ripped, torn or otherwise effected by the fall
  • Take pictures of the physical injuries – bruises, scrapes, cuts incurred as a result of the fall

Physical Evidence

Save the shoes and clothing you were wearing when the accident occurred.  Place the items in a plastic bag and mark it accordingly


  • Take contact information from relevant witnesses including their name, address and phone number
  • Survey the area where the accident took place to determine if there were any cameras that may have recorded the incident
  • Be sure to fill out an accident report with the store or business at which you were injured,
  • Take any employee’s information who responded to the incident, including their name and employee number
